New Patient Information and Requests

Kelly Collaborative Medicine makes a point to deliver accurate and personalized care services. Thanks to our training, experience, and empathetic approach to treatment, we can adapt to your specific needs. In order to get the most out of your healthcare experience, we ask that you take the following new patient procedures into consideration. After reviewing the following topics, please contact us to request your first appointment, and be sure to request a new patient form.


What to Expect During Your First Visit

Every new patient to our practice is required to fill out new patient forms. This packet is used to gather basic information about you, as well as your recent medical history. All information is kept private in accordance with state, federal, and professional guidelines. The forms must be completed before your first visit.

Your time is valuable. Our team is dedicated to respecting that fact. During your first visit, you can expect to spend 30 minutes with us. This provides us ample time to get to know you, as well as your specific health goals. As a team, we discuss your concerns, as well as draft preliminary ways to address your current state of health.

In order to make your first visit as seamless as possible, we ask all new patients to arrive 15 to 30 minutes prior to their appointment time. This allows our staff to verify and process your paperwork, as well as record important documentation. In addition to your new patient form, we also require the following documentation:

  • Insurance Forms/Proof of Insurance
  • New Patient Forms
  • Identification
  • Any Prescriptions You Are on, If Any
  • Copay

Patient Forms

Preferred Contacts (PDF) - Patients are encouraged to complete and return the Preferred Contacts Form but it is not required. Contactos Preferidos
